The AA1A is the paper claim form for Attendance Allowance. It asks for information about your medical conditions, how they affect you, and the support you need with daily activities like dressing, washing, eating, and moving around.

You won’t need this form if you are under State Pension age - in that case, you may need to claim PIP instead - or if you’re already receiving Attendance Allowance and are not reporting a change of circumstances.
