The ESA1 is the paper form used to claim Employment and Support Allowance. It’s an alternative to applying online or by phone, and it asks for personal details, medical information, and work history.
Three main reasons:
You won’t need it if you’re applying for ESA online or by phone, or if you already receive ESA and are just reporting a change of circumstances.
You should submit it as soon as you become eligible to claim ESA to avoid losing entitlement from earlier dates.
Post it to the address provided in the claim notes or take it to your local Jobcentre Plus.
