Form AA4/8 is used by individuals to apply for an Italian tax code, to report changes to their personal or residence data, and to request the tax code card or a duplicate health card.
The tax code is usually assigned automatically, for example to newborns, and to foreign nationals who apply to enter Italy for work or family reunification or who apply for a residence permit. If you do not have one, you apply with this form. You can also use it to correct or update your personal data.
Your personal details, place and date of birth, residence or domicile, and a valid identity document. The form is organised in sections for the type of request, personal data, residence and, where relevant, a person authorised to submit it on your behalf.
You can submit it in person, or through a delegate, at a Revenue Agency office (by appointment), by certified email, or online through the Revenue Agency's document submission service with a digital identity.
