What is Form D4?

Form D4 is the DVLA medical examination report completed by a doctor (usually your GP or an occupational health physician) to confirm you meet the higher medical standard required to hold a Group 2 driving licence — the category covering lorries (HGV/LGV, categories C and C1), buses and coaches (PCV, categories D and D1). D4 provides the detailed medical evidence that supports the declarations you make on Form D2, the main application form for a Group 2 licence.

When do you need Form D4?

You need a D4 medical when applying for your first HGV, LGV, PCV licence; when renewing your Group 2 licence at ages 45, 50, 55, 60, and 65, and then every year after 65; when the DVLA specifically requests one because of a declared or discovered medical condition (diabetes treated with insulin, epilepsy, heart conditions, sleep apnoea, and others); and often when applying for a taxi or private hire vehicle licence, since many local councils require a D4-standard medical for licensing even though taxi drivers hold a standard Group 1 licence.

What does the medical examination cover?

The doctor checks your vision (including a formal eyesight test to the Group 2 standard, which is stricter than the standard car licence requirement), blood pressure, cardiovascular health, and asks about your medical history — including any conditions affecting alertness, cognition, or physical capability relevant to driving large vehicles. If you have a relevant medical condition, the doctor documents it on D4, and the DVLA's medical team reviews the report before deciding whether to issue, renew, or restrict your licence.

How do you get Form D4 completed and submitted?

Book an appointment with your GP or a private medical examiner who offers DVLA D4 medicals (private options are often faster than waiting for a GP appointment). Bring photo ID and a list of any medications you take. The doctor completes and signs the D4 form, which you then submit to the DVLA along with your D2 application (for a new licence) or as part of your renewal paperwork. Processing times vary; book your medical well before your licence renewal date to avoid a gap in your ability to drive for work.
