Form DS1, Cancellation of entries relating to a registered charge, is the deed HM Land Registry requires to remove a mortgage or legal charge from a property's title once the loan it secured has been fully repaid. It's completed and signed by the lender, not the borrower, since the lender is the party giving up their registered interest in the property.
Most lenders now send an electronic discharge, known as an e-DS1, directly to HM Land Registry through the lender services portal, so no paper form changes hands at all. If your lender doesn't use this route, they'll send you a completed and signed paper DS1, or a letter confirming the mortgage has been redeemed, which you then forward to HM Land Registry yourself alongside a completed Form DS2.
Form DS2 is the application that actually asks HM Land Registry to update the register once it has the DS1. It's sometimes printed on the reverse of the DS1 itself. If you've only been sent the DS1, you can download DS2 separately from GOV.UK, or send a covering letter with the DS1 asking HM Land Registry to update the register and confirming the address for correspondence.
No, there's no HM Land Registry fee for cancelling a charge using DS1 and DS2. Evidence of identity is generally required for the parties involved unless a conveyancer is acting for them, since removing a charge without a conveyancer involved is one of the situations HM Land Registry treats as carrying fraud risk. If the borrower was a company or LLP, it's also worth telling Companies House using Form MR04, even though this isn't a legal requirement.
