Form DTT 953 I is the communication under article 331 of the Italian road code regulations (decree 495/1992) that the doctor sends to the licensing authority after a medical examination confirming that the driver is fit to renew their licence.
The doctor who carried out the examination completes and signs it. It records the driver's personal data, the date of the visit, the licence category and number, and the authority that issued the licence.
No prescription, or an obligation to wear glasses, contact lenses or a hearing aid, plus any notes.
The form must be sent to the licensing authority's central office in Rome within 5 days of the medical visit. The authority then mails an adhesive sticker for the licence to the driver's home. While waiting, the driver can keep driving with the medical certificate kept together with the licence.
