Form G-1450 is a credit card authorization document that accompanies your USCIS application. It collects the applicant’s and cardholder’s information to charge the appropriate filing fee for an immigration benefit request, such as Form I-90 (Green Card replacement), N-400 (Naturalization), or I-130 (Petition for Alien Relative).
